In the web portal, Events can be viewed, edited, and closed out. 


Start by logging into the Incidents app:

  1. Choose to view All Events (assigned to anyone in your Organization), or events Assigned To Me. Please note: This option is based on user permissions.
  2. Select an Event Type(s).
  3. Choose to view All, Open, or Closed Events.
  4. Click Apply Filters.
  5. Use the filter icons on the column headers to filter the list. You can drag-and-drop the columns in the order you prefer.
  6. Click an Event ID.

Now, review the details:

  1. Use the section tabs to jump quickly to a specific section.
  2. No matter how far down you scroll, the Form will always be visible on the right. A Form will be visible if this Event was recorded in the mobile app.
  3. View any Attachments and Media attached to this Event.

Now, begin reviewing/completing the General section:

  1. View/change the Event Type.
  2. View/change the Category.
  3. View/change the Subcategory.
  4. View the Description.
  5. Optional: Select who Reported this Event.
  6. View the Date this Event was reported.
  7. View the Date/Time this Event occurred.
  8. View the Severity.





Continue to the next field:

  1. Optional: Enter the Initial Actions taken once this Event occurred.
  2. Optional: Enter the Preceding Activity (what was occurring before this Event occurred).
  3. Optional: Enter the Address where the Event occurred.
  4. Optional: Enter a Location Description.
  5. View the Facility.
  6. Optional: Select the Department.






Next, view/complete the Status section:

  1. View who this Event is Assigned To.
  2. View the Due Date (this can be adjusted later if needed).
  3. View the Status.






If applicable, view/complete the Person Involved section:

  1. Select the Person Involved.
  2. Enter their Shift and Start Time.
  3. Enter Days Away From Work (that were due to this Event).
  4. Enter Days on Job Transfer (that were due to this Event).






If applicable, view/complete the Injury/Illness section:

  1. Toggle Person Injured.
  2. Enter the Injury/Illness Description.
  3. Optional: Select an Illness Type.
  4. Optional: Select an Injury Location.
  5. If applicable, toggle Death Occurred, and enter Date Death Occurred.






If applicable, view/complete the Treatment section:

  1. Toggle Treatment Received.
  2. Enter the Initial Treatment.
  3. Enter what the person was Harmed By.
  4. If applicable, toggle Treated Off Site. Treatment Facility, Physician Name, Treated in Emergency Room, and Hospitalised Overnight will appear as available fields.








Next, view/complete the Findings section:







Next, view/complete the Actions section. Click New.







Enter the Action details:

  1. Select if this is a Corrective or Preventive Action.
  2. Select a CAPA Category.
  3. Enter the Problem.
  4. Enter a Resolution (how this should be resolved).
  5. The Owner will pre-populate based on the selected Category. However, this can be adjusted if needed.
  6. The Assigned To will pre-populate based on the selected Category. However, this can be adjusted if needed.
  7. The Due Date will pre-populate based on the selected Category. However, this can be adjusted if needed.
  8. Select the Priority.






You can click View In Actions Portal to view all Actions related to this Event.








If applicable, view/complete the Witness Statements section. Click New.







Enter the statement details:

  1. Select the Witness.
  2. Select the Date & Time this statement was given.
  3. Enter the Statement.







Next, view/complete the Followers section. This will pre-populate based on the Event Subcategory, however, you can add/remove Followers here.

  1. Edit the Followers.
  2. Click the three dots.
  3. Save this Event. You can click on this menu again to Notify Followers or generate an OSHA report.







A report can also be generated from the Events screen. Simply highlight the Event, click Reports, and select the format you'd like to use.
