Forms are the questions users must answer for each Incident/Hazard/Near Miss. You can have multiple forms for different types of Incidents/Hazards/Near Misses.
- Click Forms.
- Select Forms.
- Click New Form.
Now you can enter the details:
- Enter a Title.
- Click the green plus sign to add a question.
Now, you can add questions:
- Select or type a Section for this question. Questions can be split into sections, or all placed into one section.
- Enter a Prompt. This is the question the user will see.
- Click the three bars to select a Response. This is how the user gets to respond to the question.
- Optional: Select if the question is Mandatory.
- Optional: Enter the Regulatory Code relating to this question.
If applicable, you can add an image that relates to this prompt:
- Click Image.
- Click the three bars to add an image.
You can also re-order the questions by dragging and dropping them. When you are finished, click Publish.
Now, you can attach this Form to a Subcategory.
- Click Settings.
- Click Incidents.
- Select a Category.
- Click the pencil to edit a Subcategory.
- Select the Form to be used.
- Click Save.
Please note: Users in the mobile app will need to Sync to see these changes.
